Ultrasonic A Scan

The "A" stands for Amplitude. It emits a beam of high-frequency ultrasound (typically 10 MHz) along a single path (one-dimensional) through the eyeball. When the ultrasound waves encounter interfaces of tissues with different densities (such as the anterior surface of the cornea, the anterior and posterior surfaces of the lens, and the retina), they produce echoes. The instrument converts the amplitude (intensity) of the echoes and the return time into a one-dimensional waveform. The height of the peaks represents the strength of the echo, and the distance between the peaks represents the distance between the tissue interfaces. OD1-A is a contact Ultrasonic A scan; during the examination, the probe gently touches the cornea (after topical anesthesia), and the speed is fast, but the pressure on the cornea may lead to shorter measurements. A-scan is also one of the important instruments for measuring axial length.

A Scan Pachymeter

An ophthalmic ultrasound pachymeter, often simply called a corneal pachymeter or ultrasonic corneal thickness meter, is a device specifically designed for accurately measuring the central thickness of the cornea. A high-frequency ultrasonic probe (typically 20 MHz or higher, a frequency higher than that of a standard A-scan ultrasound for higher resolution) is vertically aligned with the center of the cornea. Sound waves propagate from the anterior surface (epithelium) to the posterior surface (endothelium), producing two main echo peaks. The instrument calculates the time difference between these two peaks, combined with the known speed of sound propagation in the corneal tissue, to automatically calculate the precise thickness of the cornea, measured in micrometers (µm).
